I hereby grant to Mercer University, its agents and those acting on their behalf (collectively referred to as “Mercer University”), the absolute and perpetual right and permission to use my physical likeness, name, and/or voice in whole or in part, or in composite, distorted in character or form, in color or otherwise throughout the world, through any media, for the purpose of art, advertising, promotion or trade, or any other lawful purpose whatsoever.
I further grant to Mercer University, its agents and those acting on their behalf (collectively referred to as “Mercer University”), the absolute and perpetual right and permission to use the below referenced photo and video footage and that I am the sole owner of all rights in and to the photo and video footage, have the right to enter into this agreement, and that the consent of no other person, firm, corporation or organization is required for the usage purposes enumerated herein, and that the use thereof will not violate the rights of any other third parties.
I waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo or video footage.
I hereby hold harmless and release and forever discharge Mercer University from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

Fee: Gratis
I UNDERSTAND THAT NO CHANGE, ADDITION, DELETION OR MODIFICATION IN ANY MANNER TO THIS CONTRACT IS VALID OR ACCEPTED BY Mercer University, WITHOUT THE WRITTEN CONSENT BY SIGNATURE OR INITIALS OF Mercer University.
